Members of CUPE 3903 Unit 1, Unit 2 and Unit 3 who have medical or other exceptional circumstances precluding their return to work should login below and complete the template within 24 hours of the provincial legislation coming to effect to explain your circumstances.
Medical documentation may be required at a later date. If you are out of the country or otherwise unavailable, please describe your reasons,
below, including when you will become available so that we may determine how to proceed with your course obligations and your employment.
